Basic Data
This tab in a patient's record holds everything that identifies the patient. Sophia uses this information for billing, communication, and to meet legal requirements.
Personal Information
- First and last name.
- Birth date. Age is calculated automatically.
- Gender, civil state, nationality, and employment situation.
- National number and eID card number.
- Where applicable, a deceased date and an autonomy indicator.
Address
Street, house number, zip code, and city.
Contact
Phone number and email address. These are used for confirmation and reminder emails for appointments.
Other Contact Persons
Add family members or other people to contact regarding this patient.
Insurance Information
Register one or more insurers for the patient, and mark one as the default.
Settings
Turn the following on or off per patient:
- The preference scheme.
- The third party payment scheme.
- Confirmation and reminder emails for appointments.
- The use of eAgreement for this patient, if this feature is active.
Mutuality and OCMW
Name, number, and address of the mutuality, plus an OCMW registration number where relevant.
