Adding Activities to a Care Plan
Adding an activity to a care plan is one of the less obvious features in Sophia, mostly because you can also save that same activity as a patient comment at the same time, in two steps at once. This guide explains both parts separately, in plain language.
First, the Basic Idea: Why Is This Combined?
A care plan tracks what you do with a patient, in the context of a certain condition. An activity is a concrete action within that plan, for example "shoulder exercises performed."
At the same time, separately from care plans, Sophia keeps general notes about a patient on the Comments tab. Often you want to save exactly the same text in both places. To avoid typing this twice, Sophia lets you do this in one motion.
Adding an Activity, Step by Step
Step 1: open the care plan
Go to Medical Record, and click the care plan you want to add an activity to, to expand it.
Step 2: start a new activity
Click the plus icon next to Activities, within that expanded care plan.
Step 3: describe the activity
Fill in the performed activity field: a free text description of what you did. This field is required: without this description, you can't continue.
Step 4: optionally link an appointment
Want to link this activity to a specific appointment of the patient? Do this here. This field is completely independent of the description: you can fill in either one, or both at once.
Saving the Activity as a Comment Too
You only see this option when creating a new activity, not afterward.
Step 1: tick the option
Tick Log to comment logbook.
Step 2: choose a comment type
- Free text: Sophia simply reuses the description you already filled in for the activity.
- Contact report, Anamnesis, or Interpretation: Sophia opens the same full form you know from the Comments tab, so you can add more detailed information than just the short activity description.
Step 3: save
Click the save button.
What Happens Behind the Scenes?
Sophia actually does this in two steps, automatically, one after the other:
- Sophia first creates a new comment, with the information you just filled in.
- Sophia then links the activity to that comment.
You don't notice any of this yourself: you simply get one saved activity, and one new comment, which you'll find among the patient's other comments.
WARNING
This option is only available when creating a new activity. Do you edit an existing activity afterward? Then you no longer see this checkbox, so you can't tick it retroactively.
Common Mistakes
- Thinking you can still save the activity as a comment later: this is only possible when creating it, not when editing it.
- Forgetting that "Free text" just reuses your existing description: want more detail in the comment than in the activity, choose a different type instead.
- Linking an appointment without filling in a description: this doesn't work, the description always stays required.
